Preparing for Your Visit

Ready to get started? Here’s what to expect:

1. Do you need a prescription? In California, you can begin physical therapy without a referral (this is called direct access). This allows you to receive treatment from a licensed physical therapist for up to 45 days or 12 visits without a prescription.

Some insurance plans do require a physician referral for reimbursement. If you’re unsure, I can help you determine what your plan requires.

2. Schedule your first session Call (323) 965-7713 or email diana@reisspt.com to schedule your initial evaluation. Email is usually the fastest way to reach me.

3. Insurance information If you plan to use insurance, please email a photo of the front and back of your insurance card, along with your date of birth. This information is required to verify your coverage.

4. Set up your space On the day of your visit, wear comfortable clothing such as shorts, sweats, or a t-shirt. Choose a space where you can sit, stand, and lie down comfortably, and where you can be clearly seen and heard on video.

If available, you may want to have items like an exercise mat, resistance bands, foam roller, or tennis ball nearby. These are helpful but not required. During your evaluation, I will guide you on what may be useful for your specific needs.

5. Complete your forms Please return your new patient forms prior to your first session by emailing them to diana@reisspt.com.
